Active 4 Life LLC — Registration Terms of Agreement

Instructions
This terms of agreement and release of liability is intended for individuals participating in the summer camp offered by Active 4 Life LLC (“A4L” or “Camp”). “We” refers to both the enrolled student and the Participant’s parent or legal guardian.
SPARTS Camp Cancellation and Refund Policy
Full refunds (minus credit card processing fees) will be issued for cancellations made at least 2 weeks from the start of camp. No refunds will be issued for cancellations made under 2 weeks from the date of registration.
Behavior Guidelines
Campers are required to behave in a mature, responsible manner. Core values: Responsibility, Respect, Honesty, Care. Camp fees are non-refundable for disciplinary dismissals. Physical violence or bullying results in immediate dismissal.
Assumption of Risk
We acknowledge Camp involves physical activity with inherent risks. By registering, we assume all risks inherent in the Participant’s participation.
Release of Liability and Agreement Not to Sue
I release Active 4 Life LLC, its owners, employees, agents, and assigns from any and all liability arising out of or related to the Participant’s participation. This is a binding legal agreement.
Indemnity
I agree to indemnify and hold harmless A4L from any losses or costs arising from injury caused by me or the Participant while participating in the Camp.
Participant Health
I affirm the Participant is in good health. In a medical emergency I authorize A4L staff to arrange emergency care as they deem necessary.
Program Changes
A4L reserves the right to make changes or cancellations. In the event of full Camp cancellation, A4L will provide a refund.
Photo Release
I grant Active 4 Life LLC permission to photograph/video the Participant for promotional materials, unless I request otherwise in writing.
Other Terms
Subject to the laws of Washington State. Disputes settled in King County, Washington.
Registration Acknowledgement
By proceeding, I confirm I am the parent or legal guardian and agree to all terms above to the maximum extent permitted by law.
